Supportive Home Care The Black River Industries, Inc. (BRI) Supportive Home Care (SHC) Program provides support to eligible individuals with disabilities (participants) who reside in unsupervised living environments such as their own apartments or homes. The program's purpose is to enable participants to maintain independence in the community. OBJECTIVES: Provide a range of services for participants who require assistance to meet their daily living needs, ensure adequate functioning in their home, and permit safe access to the community. PLACE OF PROVISION OF SERVICES: Services are offered in participants' homes, as well as at relevant sites in the community. METHODS OF PROVISION OF SERVICES: Services promote participant choice, are based on the participant's needs, and are provided on a one-to-one basis by trained staff. PROGRAM DESCRIPTION: Supportive Home Care Services are individualized and participant-driven based on each unique case, and may include some or all of the following components: Personal Services:
Household Services:
ELIGIBILITY: Eligible participants are those who need assistance in order to maintain their ability to live independently. HOURS OF SERVICE: Five (5) days a week (Monday-Friday) as activity dictates. Services will primarily be provided between 7:00 AM and 8:00 PM, though will be flexible as necessary. Weekend service is available when needed.
