Supported Apartment / Daily Living Skills Training Program Statement
The Black River Industries, Inc. (BRI) Supported Apartment Program provides support to eligible individuals with disabilities (participants)
who reside in an unsupervised living environment such as their own apartments or family homes. The
program's purpose is to assist participants to achieve and/or maintain independence in the community.
1. Provide participants with training supports as necessary to ensure a successful transition from a supervised living environment to that of independent living.
2. Provide participants with daily living skills training, as well as access to community supports as a means of fostering integration into the community.
3. Provide those participants who are living independently with the ongoing supports needed to maintain independence.
PLACE OF PROVISION OF SERVICES:
Services are offered in the participant's home, as well as at relevant sites in the community.
METHODS OF PROVISION OF SERVICES:
Services promote participant choice, are based on the participantâ€™s needs, and are provided on a one-to-one basis by trained staff.
Services are individualized and participant-driven based on each unique case, and may include some or all of the following components:
Assessment- A broad functional assessment of the participant's strengths and needs is available. This assessment may occur in the community as well as the residence of the participant, and will provide the basis for the participant and staff to develop an Individualized Service Plan.
Training- Skills training will be offered in both the residence and in the community in areas identified in the service plan. These may include, but are not limited to: housekeeping; meal planning/nutrition; budgeting; problem solving; hygiene/grooming; task focus and completion; planning and managing activities; accessing community services; stress management; socialization and interpersonal behavior.
Support Services- Skills training may continue to be provided in some of the above areas until adequate skills are developed to perform them independently. Participant success in independent activities will also be monitored, with support or retraining provided as needed.
Eligible participants are those who have the potential to learn independent living skills.
HOURS OF SERVICE:
Five (5) days a week (Monday-Friday) as activity dictates. Services will normally be provided between 7:00 AM and 10:00 PM, though will be flexible as necessary. Weekend service is available, primarily in emergency situations.
