Day Services and Community Integration Program Statement
The Black River Industries, Inc. Day Services Program is a multi-faceted program offering adults with severe and profound disabilities and special needs a broad range of activities, training, and personal care services. It is designed to maximize individual functioning, with the ultimate goal of increasing independence and community integration for all clients.
1. Provide recreational and leisure activities. 2. Provide individual programming to facilitate gross and fine motor skill development. 3. Improve individualized daily living skills training. 4. Increase self and environmental awareness. 5. Improve communication skills. 6. Provide personal care services. 7. Provide sensory stimulation including tactile, visual, and auditory activities.
PLACE OF PROVISION OF SERVICES:
The Day Services Program meets at Black River Industries, Inc. and in the community. Black River Industries, Inc. is a handicap accessible facility.
The staffing pattern will provide a minimum of two direct staff to every fifteen day services program participants. The staff to participant ratio will be increased based on the needs of specific persons served.
METHODS OF PROVISION OF SERVICES:
The program is supervised by the Day Services Program Manager and a trained staff of program aids.
The Day Services Program is designed to provide adults with severe and profound disabilities and special needs with individualized training, personal care, and activities that enhance their daily life. Transportation is also offered, within Taylor County, to facilitate each client's individualized needs. The focus is to assist these individuals towards maximizing their potential and maintaining it in functional activities, personal care, communication, and awareness of self and the environment. A broad functional assessment of the individual's strengths and needs is available. The assessment will provide the basis for the individual and his or her family and/or guardian and Black River Industries' staff to develop an Individualized Service Plan that will direct daily programming. An initial program review is offered after 30 days from the start date. After the initial review an annual program review is provided on a six-month basis or, more frequently, as indicated.
The Day Services Program serves adults with severe and profound disabilities with special needs and limitations.
HOURS OF SERVICES:
Monday through Thursday - 8:30 AM to 4:00 PM.
